STEP 1: Click on the link here to take you to my PPD landing page.
STEP 2: Click on the REQUEST FOR APPOINTMENT button.
A pop up window will appear for you to either Log in (if you have an existing account)
or Register (by creating an account).
STEP 3: Select the date you want to schedule the appointment.
STEP 4: Download the PPD for Patients app.

The download link will also be sent to your email and/or phone. We will use this app to communicate during our teleconsult.
STEP 5: Fill in the CONSENT FORM seen below and click submit.
STEP 6: Chill. ;) Wait for your appointment date. Wait for the message that comes with a unique link. you just need to tap that link and you are ready to answer the call.
